I had a call with a 67-year-old patient with psoriasis who was on a high dose of Zyrtec and ended up with a full-body rash, but thankfully no signs of anaphylaxis. Their vitals were stable, and their lungs sounded clear. I thought about giving them Benadryl for the itching, but I held back because I was worried it might make things worse. The base didn't give any clear advice. We started IV fluids, kept a close watch, and transported them.
What would you have done in this situation? Would you have given them antihistamines or played it safe? I'd love to hear about your protocols and experiences.
